1. Completed Application Form.
2. Completed Affidavit of Financial Support (with your signature) for international students.
3. A money order or cashier's check: $55.00 for international students and $40.00 for U.S. citizens and permanent residents.
4. Signed Confirmation Page.
5. GRE scores.
6. TOEFL scores for international students.
7. Statement of Objectives.
8. Resume.
9. Official transcripts and their English translations if the original documents are not in English.
10. Three letters of recommendation. Please click here for Recommendation Form (pdf).  This form MUST be used for recommendations.
11. Diplomas (or copies) for previous college degrees (BS and MS) and their English translations if the original documents are not in English.
